Office Policy

Appointment Arrival

We ask that patients arrive 15 minutes prior to their scheduled appointment so that we can be sure to have them and everything ready by the time the appointment is scheduled to start.

Online Registration

Usually about two days prior to your scheduled appointment we will send you a secure, HIPAA compliant, link to fill out your online registration forms. You will not need to print anything out to bring with you. You will be able to simply input all your information online. Once you press submit your information will be sent directly to our system. We can either text or email you this link depending on your preference. We ask that you fill out these registration forms prior to your appointment to ensure that you are able to get into your appointment on time and keep things moving smoothly.

Canceling an Appointment

If you need to reschedule your appointment for any reason we ask that you please call our office at least 24 hours prior to the scheduled appointment time so that we may be able to give that appointment to another patient who needs it. 


Payment is due at the time of service. When you schedule an appointment our office will give you a rough estimate of what we believe your insurance will cover and what you will be responsible for. We will then take a look at your specific plan and get a more accurate estimate. We will typically provide you with this updated estimate about two days prior to your scheduled appointment. Payment of your estimated portion will be due at the end of your appointment. Once we receive payment from your insurance we will either send you a bill or a refund depending on what your insurance company paid.
